Mauri Ora Navigator (Rheumatic Fever)

Te Rūnanga o Toa Rangatira is looking for a Mauri Ora Navigator – Rheumatic Fever to join our team and work alongside whānau across the Ngāti Toa rohe.

Ko te tūranga mahi | About the role

As a Mauri Ora Navigator, you will work alongside whānau, tamariki and rangatahi to provide practical, culturally responsive and whānau-centred support.

You will help whānau navigate the health system, connect with the right services and understand how they can prevent rheumatic fever and support their own health and wellbeing.

Your mahi will include:

  • Building trusted relationships with whānau through whanaungatanga and manaakitanga.

  • Supporting whānau with rheumatic fever prevention, early intervention and follow-up.

  • Promoting awareness of sore throats, Group A Streptococcus and the importance of timely treatment.

  • Supporting whānau to understand and complete antibiotic treatment.

  • Connecting whānau with health, housing, Whānau Ora, education and social services.

  • Advocating for whānau and helping remove barriers to accessing care.

  • Supporting health promotion and community education.

  • Working alongside health providers, schools, community services and other agencies.

  • Bringing whānau voice and experience into service improvement.

You will be someone who:

  • Is passionate about working alongside Māori whānau and communities.

  • Has a strong whānau-centred approach.

  • Understands and demonstrates manaakitanga, whanaungatanga and mana motuhake.

  • Has strong communication and relationship-building skills.

  • Can confidently navigate health and social services.

  • Is comfortable working with whānau experiencing complex health and social circumstances.

  • Can advocate for whānau and build effective relationships across different services.

  • Works well independently and as part of a team.

  • Is organised, proactive and able to manage competing priorities.

  • Understands the importance of Te Tiriti o Waitangi and equity in health.

  • Is committed to learning and developing their understanding of Ngāti Toa tikanga, kawa, whakapapa and aspirations.

Experience in health navigation, Whānau Ora, primary/community healthcare, social services or working with tamariki, rangatahi and whānau would be an advantage.

Knowledge of rheumatic fever or Group A Streptococcus is desirable, but we are also interested in people who bring strong community connections, the right values and a genuine commitment to whānau wellbeing.
